Vendor Risk Assessment Template

Six weighted risk categories, auto-calculating weighted scores, a total risk score, and a score-interpretation guide.

Why weighted scoring beats a gut-feeling rating

A single "how risky is this vendor, 1 to 10" question invites inconsistency — one evaluator's 7 is another's 5, and neither of them can point to exactly why. This template breaks risk into six specific categories, each scored independently and weighted by how much it typically matters, so the final number reflects an actual structured evaluation rather than a single person's impression on a given day.

  • Financial stability (20%) — credit rating, payment history, public financial signals
  • Compliance & certifications (20%) — required licenses, insurance, industry certifications current
  • Delivery reliability (20%) — on-time delivery history, capacity to scale
  • Data & security practices (15%) — data handling, access controls, breach history
  • Business continuity (15%) — backup suppliers, disaster recovery plan, geographic risk
  • Reputation & references (10%) — client references, public complaints, litigation history

Score each category 1 (high risk) to 5 (low risk), and the spreadsheet calculates the weighted score and total automatically.

What to actually do with the score

The template includes a simple interpretation guide: scores from 4.0–5.0 indicate low risk and standard onboarding is fine; 2.5–3.9 suggests moderate risk worth additional monitoring or a shorter initial contract term; below 2.5 should trigger a leadership review before proceeding. These thresholds are a starting point — the value of scoring vendors consistently is being able to compare them against each other and against your own past assessments, not hitting an exact universal cutoff.

This kind of structured risk tracking is exactly the piece that tends to fall apart in spreadsheets once a vendor list grows past a couple dozen — nobody remembers to re-run assessments on schedule, and risk scores go stale without anyone noticing. Our vendor management platform features overview covers how that tracking typically gets automated once manual review no longer scales.

FAQ

Common questions about this template

Each of the six risk categories has a weight (totaling 100%). You score the vendor 1 to 5 per category, and the template multiplies score by weight automatically to produce a total weighted risk score out of 5.

Yes. The default weights are a reasonable starting point, but you should adjust them if one risk area — data security, for example — matters more for your specific vendor relationships.

Annually at minimum, and again whenever a vendor's contract comes up for renewal, since risk profiles can change meaningfully over the life of a relationship.
